To The Rescue’s supportive community living (SCL) residential medical homes are ideal for individuals with health related challenges. Currently located in Cedar Rapids and Marion, our fully accessible, five bedroom residential medical homes are ideal for individuals with intellectual disabilities and persistent medical needs. Our homes are designed to meet the unique accessibility and medical needs of each member. Services are provided by staff specially trained to work with individuals with intellectual disabilities who also have medical needs.

As part of our comprehensive life services model, eligible members living in a To The Rescue Medical Home have the option of receiving our home health care services. The To The Rescue Home Health team of nurses are an integral part of the medical home team, working closely with members and their physicians to develop and follow an individualized plan of care. To The Rescue’s comprehensive home health services include home health nurses, physical/occupational/speech therapists and home health aides
