To The Rescue Medical Housing is designed for individuals who require on-site care for complex medical needs. Our medical homes offer a unique opportunity for individuals requiring comprehensive, high level medical care to live in a safe, secure home while enjoying the benefits of fully integrated programming unique to To the Rescue.

Trained Care Staff

At To The Rescue we understand your desire to find the best, supportive medical care for your loved one.

To The Rescue offers an environment that feels like home, and provides required medical support. Each individual’s needs are unique and important, which is why we staff a multidisciplinary team. 

By partnering with our home health division, our trained staff customizes medical services needed for each of our residents. To The Rescue nursing, aide and direct support staff work together providing daily living and medical assistance for residents’ individualized needs. 

We prioritize on-going education and staff training specific to each resident. Our home health clinical nurse educator trains To The Rescue housing personnel, ensuring each resident’s individual medical needs are skillfully managed.

Our trained staff includes:

  • RN Clinical Nurse Educator – medical oversight and staff training
  • LPN – medical care
  • CNA / Home Health Aide – bathing assistance
  • Direct Support Professional- manage, care, support and supervise daily living

With communication and care, we strive to provide peace of mind, reassuring you that your loved one is safe, secure and supported.

Accommodations

To The Rescue Medical Home residents enjoy universally designed, zero entry, one-floor living with a private bedroom, a shared living area, and an accessible outdoor living space. This environment provides privacy and encourages socialization for up to five residents per home. Accessible transportation services, day programs and supported employment options encourage inclusion and community participation. Our members stay active and busy with outings, events, and volunteer opportunities.
