Host Home Provider

To The Rescue Colorado works with individuals who would prefer a placement outside of the family home in a host home.  To The Rescue offers a wide variety of host home options based on client preference. Our homes are in neighborhoods close to community resources that include shopping, transportation and entertainment.

To the Rescue Colorado collaborates with families, individuals receiving services, and case managers to develop person centered solutions that meet the goals and preferences of our clients.  New residential placements are monitored closely to ensure a good match between host home family and client.  Open communication is stressed; alternative placements are available for individuals who request a move.

Our host home residents enjoy community living while receiving the support and supervision they require to remain happy and safe. Individualized support is available for activities of daily living such as meal preparation, medication administration, community activities, doctor visits, and transportation.  Individuals  in our residential program are actively involved in planning their daily routines and the services they need to be successful.

A host home helps individuals live life to the fullest in a family setting. Care is taken to match a host home with the individual to ensure that a meaningful relationship can develop. The goal is to create bonds of friendship and family while providing needed support.

To The Rescue Colorado Host Homes are a unique living arrangement where an individual or family in the community opens their home to a person with an intellectual or developmental disability. Host homes provide individuals with a sense of family and community. Through this supportive living option, individuals receive the personalized services they need to live independent, joyful lives in a home-based setting.

Who is eligible to live in a host home?

Individuals that receive services through the DD waiver program are eligible to live in a Colorado host home.

Meet Christopher

Prior to joining the To The Rescue Residential Team, Christopher was living alone in an independent housing. As time went by, his health deteriorated because he was unable to attend scheduled doctor’s appointments, nor could he keep his apartment clean. To obtain the additional support Christopher needed to improve his life, he connected with To The Rescue and the To The Rescue Host Home Program. How do I become a host home provider?

Host home providers are independent contractors who are paid a per diem rate for the supervision and care of an adult with a disability; they also are paid a monthly fee for room and board.

If you are thinking that becoming a host home provider may be for you, or if you have questions, one of our residential experts would be happy to walk you through the process and answer any questions you may have. Generally speaking, the primary host home caregiver must:

  • pass a background and CAPS check, as well as other
    individuals 18+ years of age residing in the home
  • have a valid driver’s license and vehicle
    insurance, as well as a vehicle that passes all
    safety checks
  • provide professional references

After background and other checks are completed, a To The Rescue staff member will walk you through the process to ensure that your home meets all requirements to safely serve an adult with a disability.

In most cases, host homes must allow for an individual receiving services to have private use of their own bedroom; however, host homes vary in type and size, from apartments to condominiums to single family homes.

What services do I provide as a host home provider?

To The Rescue supports a wide range of individuals with an array of support needs. Some of the activities you might provide as a host home provider include:

  • coordinating appointments
  • managing medications
  • providing personal care
  • providing transportation
  • preparing meals
  • organizing recreational activities
  • promoting individual independence
  • assisting with finances (as applicable)
  • communicating with the interdisciplinary team,
    including guardians and case managers
  • facilitating opportunities for community
    integration
  • completing daily provider’s notes
  • conducting monthly safety drills

What training do I receive as a host home provider?

Learn if becoming a host home provider is right for you and your family. Our team of experts at To The Rescue will assist you to ensure you are ready and able to do the job and that you have the supports you need to be successful!

Upon completion of a successful home inspection as well as successfully passing background checks, a To The Rescue staff member will guide you through the training requirements, which include:

  • initial training topics
  • mandatory reporting
  • medication administration
  • CPR/first aid training
  • annual training

To The Rescue meticulously selects host homes and ensures the host home provider has the skills needed to support the individuals we serve. To ensure success, we are available for 24/7 on call support to all host home providers.
