At To The Rescue Iowa, we offer a comprehensive array of services including day program, residential housing; hourly services; employment services, host homes, and transportation to support our mission of improving the quality of people’s lives. We specialize in providing services to individuals with developmental disabilities, brain injuries and/or mental illnesses, as well as individuals with medical needs who live in community settings. We partner with To The Rescue Iowa’s Home Health Division to offer one of a kind support for our member’s medical needs.

DAY PROGRAM

The To The Rescue Iowa Day Program offers safe and supportive services for individuals who need assistance or supervision during the day. We help individuals with disabilities develop life skills that promote independence by supporting physical and emotional health, self-advocacy, socialization and community engagement. The programs are designed to meet the unique needs and interests of all participants and offer a variety of choices.

Our comprehensive program model allows caregivers to have peace of mind meeting their loved one’s needs while balancing their own responsibilities. Our programs currently include Connect Club (ages 16-21), Adult Day Services (ages 18+), and Volunteer Opportunities. Select the program that best fits your needs!

CONNECT CLUB

Connect Club offers services for young adults ages 16 to 21. Connect Club is offered Monday – Friday all summer and during winter and spring breaks for individuals in the Cedar Rapids and surrounding school districts.

Connect Club focuses on providing opportunities to have fun, engage in new experiences and build lasting friendships in a safe and supportive environment. Participants develop important life skills they will need as they transition from high school to adult life.

In addition, To The Rescue Iowa can provide door-to-door transportation through our fleet of vehicles that includes wheelchair accessible minivans and mini buses.

There is no cost for families with students on the Medicaid ID Waiver. Please inquire if you are interested in paying privately.

24 hour residential services

ADULT DAY SERVICES

Our adult day services are designed for individuals ages 18+ who need supervised care in a safe, friendly place during the day. Participants socialize and enjoy planned activities in a group setting. At the same time, they offer family caregivers respite from caregiving duties while knowing that their loved one is in a safe place.

Programs offered through To The Rescue Iowa Adult Day Services include Adult Day Program, Social Club, and Social Scene. In addition, we host several special events each month that provide even more opportunities for individuals to socialize and make new friends.

Adult Day Program

Our adult day program is available to adults ages 18 and older. The adult day program is designed to help individuals with disabilities develop life skills that support their independence at home and in the community. We offer adult day services at two locations in Cedar Rapids, IA.

  • 2100 1st Ave NE

Each location boasts a fully accessible activity center where members have the opportunity to work on personalized goals to help gain life skills that will support living in the community with independence. Participants choose from a variety of enriching activities in and out of the center that focus on many different skills and areas, including:

supported community living

Social Club

Social Club is for individuals receiving Medicaid who have a community-based case manager that helps coordinate their services. There is no out of pocket cost to participants, and we even pick them up and bring them back! Open to individuals aged 18 and older, the purpose of the club is to have fun, meet new people, and build connections in a supportive environment. The club, located in both Cedar Rapids and Waterloo, is open Monday through Friday.

In addition to good company, participants will have the chance to join in a variety of activities that will help keep them healthy, make sure they have people to turn to when they need help, and learn about the best places to have fun in and around their community.

Social Scene

The Social Scene is designed to meet the unique interests of participants who like spending time with a small group of people or who need additional support or a quiet space. The Social Scene provides a relaxed and engaging environment where participants have the support they need to make friends, reduce isolation and combat loneliness. The Social Scene is a great option for those who have not been able to find the right fit for day program services.

Special Events

To The Rescue Iowa Day Program is pleased to host BINGO and Dance Party each month in Cedar Rapids. Bingo and the Dance Party are open to participants in all programs (some age restrictions apply). These events are free to attend.

VOLUNTEER OPPORTUNITIES

To The Rescue provides a multitude of opportunities for members to volunteer. Volunteer opportunities are ever evolving and improving. The current opportunities we provide are Hawkeye Community Area Program (HACAP), Meals on Wheels, Freedom Festival, Camp Courageous, and Assisted Living and Memory Care Facilities.

RESIDENTIAL HOUSING

Our residential housing program provides an alternative to assisted living centers. Adults who need extra supervision or care can live in the least restrictive setting while receiving the level of care and assistance they need. To The Rescue Iowa offers supportive community living homes and daily services. In addition, opportunities to provide senior housing are being explored.

Supportive Community Living Homes

To The Rescue Iowa provides 24-hour housing and services for adults with disabilities. We also offer supportive community living homes for individuals with autism or special medical needs. Our single-family homes are in neighborhoods close to community resources that include shopping, transportation and entertainment. To The Rescue collaborates with families, individuals with disabilities and case managers to develop Individualized Service Plans (ISPs) and continually adjust service plans as individuals reach new goals or their needs change.

Our members live in a small group setting (five members per house) and receive support based on need while enjoying the safety and security needed to grow in their independence. Based on need, we may offer round-the-clock, 24-hour staff assistance with meal preparation, medication administration, daily activities, and transportation.

Every individual in our supported community homes is actively involved in planning their daily routine and the services they need to be successful. Our team members provide opportunities to participate in every day activities for self discovery; life enrichment and other life skills. We can plan meals together; go grocery shopping; attend a movie; go to a ball game; see a play; visit the zoo; discover new hobbies; attend community events and many other enriching activities.

To The Rescue Iowa is opening new supportive community living homes!

We are currently accepting referrals for new supportive community living homes in Cedar Rapids and surrounding communities. Located in residential neighborhoods, these single family homes are being designed to provide tailored services and supports for individuals with disabilities.

Several of our homes focus on meeting the needs of individuals with autism. Personalized support is provided that includes ongoing opportunities for community engagement while respecting each individual’s need for structure and routine.

We are also opening medical homes to support individuals who need support with health related challenges.

Daily Services

To The Rescue Iowa supportive community daily living daily services provide support for individuals in small group settings in residential homes. Trained direct support professionals provide support 24 hours a day, 7 days a week to help individuals with disabilities live with independence and dignity. Individual choice and self-advocacy are central to the overall service.

  • Physical and mental health monitoring
  • Grocery shopping, meal planning and preparation
  • Housekeeping and laundry
  • Shopping
  • Financial management (including representative payee services)
  • Social and leisure skill development

To The Rescue Iowa accepts funding for supportive living community services, both daily and hourly, via the Iowa Medicaid Home and Community Based Service Waiver Program. The waivers we are able to accept at this time include Intellectual Disability (ID) Waiver and the Brain Injury (BI) Waiver. We also accept habilitation (HAB) funding, MHDD East Central Region funding and private pay.

HOURLY SERVICES

To The Rescue Iowa provides supported community living hourly services that help individuals with disabilities live independently in the community. These services are perfect for a loved one who may not require residential or supported community living but do need extra support for some daily activities. Trained direct support professionals (DSPs) help members in a one-on-one setting with a minimum required timeframe. Member needs are assessed with member, family, case manager and staff to determine essential learning and enrichment activities. We assist with:

  • Medication and medical monitoring
  • Physical and mental health monitoring
  • Grocery shopping, meal planning, and preparation
  • Housekeeping and laundry
  • Financial management
  • Social and leisure skill development
  • Problem-solving and coping skills
  • Crisis management
  • Home organization, decluttering skills
  • Personal care
  • And other skills to promote successful community living

Our supportive community living hourly services are provided at home or in the community. The support To The Rescue Iowa provides helps individuals achieve greater independence, foster community integration and promotes an active and healthy lifestyle.

EMPLOYMENT SERVICES

At To The Rescue Iowa, we offer employment services to individuals with disabilities who experience barriers to finding employment. We provide a range of support services, including personalized job searches, skill building and job coaching. Our goal is to help you attain financial independence and find a rewarding work experience.

Day habilitation program

HOST HOMES

A To The Rescue Iowa host home is a supportive independent living option for people with disabilities who would benefit from personalized services in a home- based setting.

An individual in the community will open their home to a person with a disability and provide services for that individual such as coordinating appointments, medications, personal care, transportation, meals and recreational activities.

Transportation for the elderly

TRANSPORTATION

At To The Rescue Iowa, we provide safe and reliable transportation services close to home, throughout Iowa, and beyond. Our customized plans — for both groups and individuals — pick you up and deliver you wherever you choose.

Whether you need a ride to work, the grocery store, a doctor’s appointment, a business meeting, or a family event, we are here to serve. You’ll appreciate our well-maintained and comfortable fleet of vehicles:

  • Minivans (seat up to 5 passengers)
  • Passenger vans (seat up to 14 passengers)
  • Wheelchair accessible minivans
  • Mini buses (seat up to 12 passengers)
  • Mini buses (can accommodate up to 4 wheelchairs)

Our fleet of vehicles includes wheelchair accessible minivans and mini-buses.

Standard wheelchairs are available upon request. We are licensed, bonded and insured.
