Daily & Hourly Supportive Community Living

Service Area: Eastern Iowa

To The Rescue provides supported community living hourly services that help individuals with disabilities live independently in the community. Our supportive community living services are provided in the home or in the community. 

To The Rescue addresses the desires, goals, abilities, needs, health and safety of each individual.  To The Rescue helps to foster community integration, seek and develop natural support and promote an active and healthy lifestyle. These services are perfect for a loved one who may not require residential or SCL living but do need some extra supports for some daily activities.

Trained direct service professionals (DSP’s) help members in a one on one setting with a minimum required timeframe. Member needs are assessed with member, family, case manager and staff to determine essential learning and enrichment activities. We assist with:

  • Medication and Medical monitoring
  • Physical and Mental health monitoring
  • Grocery shopping, meal planning, and preparation
  • Housekeeping and laundry
  • Financial management
  • Social and Leisure Skill Development
  • Problem-solving and Coping Skills
  • Crisis Intervention
  • Shopping
  • Home maintenance
  • Personal cares such grooming and hygiene
  • Transportation Training and Resources
  • And other skills to promote successful community living

An example of what this may look like for you or your loved one a

TTR staff will come into your home and help with any of the examples listed below.  

  • Staff could help make a grocery list, take you to the grocery store and assist you with your shopping to assist you with maintaining nutritional goals.  
  • Staff can assist with medication by ensuring that medication reminders are set up on your phone or calendar.  
  • Staff can assist you in planning how to declutter and create organization within your home to assist you in your cleanliness and maintenance goals.

In Iowa, To The Rescue accepts funding for Supportive Living Community services, both daily and hourly, via the Iowa Medicaid Home and Community Based Service Waiver program. The Waivers we are able to accept at this time include Intellectually Disability (ID) waiver and the Brain Injury (BI) waiver. We also accept habilitation (HAB) funding, MHDD East Central Region funding, and private pay.
